The application form can be downloaded free of charge from TIPO's website (at https://www.tipo.gov.tw/tw/trademarks/1005.html and https://www.tipo.gov.tw/tw/trademarks/588.html). Applications are divided into four different categories, namely trademark, collective trademark, certification mark, and collective membership mark. Each category has its own type of application form.
