The application form can be downloaded free of charge from TIPO's website (at https://www.tipo.gov.tw/trademarks-tw/np-533-201.html). Applications are divided into four different categories, namely trademark, collective trademark, certification mark, and collective membership mark. Each category has its own type of application form.
